Gynaecology or gynecology is the area of medicine concerned with conditions affecting the female reproductive system. So she thought the “private” investigator was an investigator of private parts.
An additional joke is that the PI is attentive enough to immediately realize she had the wrong office.
OP here, this was a joke posted in a polyglot community. I think there's a bilingual layer here as well.....
Maybe because a lot of gynecology is performed on the "downstairs" that feeds into the joke as well?

"Private" can mean genitals.
A gynecologist is a doctor who investigates "privates", and therefore could be considered a "private investigator". A "private investigator" could also refer to a person who can be hired to do detective work.
Go Go Gadget Introduction!! Inspector Gadget here from Season 16 Episode 3 of Fox’s “The Family Guy” here to expound the meaning of the joke. You see a private investigator is like a freelance detective for hire, however a persons ahem “special area” is typically referred to as their “privates”. Wowzers what a blunder!! It would appear the woman in this situation was actually looking for someone who investigates well “privates”. The woman really should see a Gynaecologist instead of some two-bit wannabe cop! Anyways I gotta go I just got a call on my handphone from Chief Quimby apparently there’s been a sighting of MAD agents here on Spooner Street, stay safe, and remember I’m always on duty!! But why is the writing on the door backwards, or rather not backwards? It was more noticeable than the joke.
Inspector Gadget here! You’re absolutely correct the writing on the door is wrong, and it should be backwards. It would seem to me that the author of the comic probably just forgot, good thing there are smarty-pants like you out there to point out their mistakes!! Anyways I’m gonna go build Brown Bricks in Minecraft!!
